A German Study Suggests the Pickled Cabbage Has a Modest Effect on Blood Pressure, but the “Live Bacteria” May Not Be the Magic Bullet We Thought

Introduction
For years, fermented foods have enjoyed a reputation as gut-health superheroes. From kombucha to kimchi, the message has been clear: eat more fermented foods to boost your microbiome, reduce inflammation, and improve your overall health. But a new, rigorous study published in the European Journal of Clinical Nutrition suggests that for healthy individuals, the reality of these benefits may be more nuanced—particularly when it comes to the humble sauerkraut. [1]
The study, a randomized crossover intervention trial conducted by researchers at the University of Freiburg in Germany, put sauerkraut to the test. They wanted to see if daily consumption for four weeks would lower blood pressure, affect inflammatory markers, and glycemic control, and whether the live bacteria present in fresh sauerkraut were essential for any observed effects.
Sauerkraut Lowers Blood Pressure and the Surprising Sodium Paradox
One of the most intriguing findings was a small but statistically significant reduction in systolic blood pressure. Participants who ate 100 grams of sauerkraut daily—whether fresh or pasteurized—saw an average decrease of about 1.5 to 2.5 mmHg.
This is a surprise because sauerkraut is traditionally high in sodium. We know that high salt intake is linked to high blood pressure. The fresh sauerkraut used in this study had a salt concentration of 0.8%, while the pasteurized version had 1.2%. And yet, blood pressure went down.
A decrease of 1.5 to 2.5 mmHg may seem modest, and in absolute terms, it is. But from a public health perspective, even small reductions matter. Epidemiological studies have estimated that a sustained 2 mmHg drop in systolic blood pressure across an entire population could reduce the risk of stroke by about 10% and heart disease by about 7%.
When multiplied across millions of people, these seemingly tiny changes translate into thousands of preventable cardiovascular events each year. So while this effect alone won’t transform an individual’s health overnight, it represents a meaningful contribution to a heart-healthy lifestyle.
The lower blood pressure despite the sodium content of sauerkraut suggests that the blood pressure benefits of sauerkraut may be driven by something other than, or perhaps outweighing, the sodium content.
Researchers suspect that the secret may lie in bioactive compounds produced during fermentation, such as ACE-inhibitory peptides. These compounds are known to help relax blood vessels and lower blood pressure, a phenomenon also observed in other fermented foods like soybeans.
Other Fermented Foods That Can Lower Blood Pressure
Sauerkraut is not alone in this apparent paradox. Kimchi, the Korean fermented cabbage dish, has shown remarkably similar results. A meta-analysis of intervention studies found that fermented kimchi consumption was associated with significant reductions in systolic blood pressure of about 3.48 mmHg and diastolic blood pressure of about 2.68 mmHg—despite also being a high-sodium food. Researchers attribute this to beneficial bacteria and bioactive compounds that appear to offset the hypertensive effects of salt. [2]
Other fermented foods have also demonstrated blood pressure-lowering potential. Fermented soybean products, including Chinese douchi [3] and Korean soybean paste, contain ACE-inhibitory peptides that can help relax blood vessels. Laboratory studies have identified specific peptides in these fermented foods that inhibit the angiotensin-converting enzyme—a key player in blood pressure regulation—similar to how some blood pressure medications work.
Fermented milk products have also shown promise, though the evidence is more mixed. A recent meta-analysis found that kefir consumption did not significantly reduce blood pressure, though longer-term studies suggest possible benefits for inflammation markers. However, other research on probiotic-fermented milk in animal models has demonstrated meaningful reductions in systolic blood pressure, likely through effects on the renin-angiotensin system and gut-immune pathways. Both Pasteurized vs. Non-Pasteurized Sauerkraut Lowers BP
The finding that pasteurized sauerkraut had the same effect as fresh sauerkraut adds another layer to the mystery. It means that the live, active bacteria (the probiotics) are not responsible for the blood pressure-lowering effect. The key is likely something that survives the pasteurization process—perhaps the organic acids, phenolic compounds, or the aforementioned peptides.
A consistent theme across these studies is that the beneficial effects are likely derived from the metabolites of fermentation, not the live bacteria themselves. This suggests that even pasteurized fermented foods—or those containing postbiotics—may offer cardiovascular benefits.
The Postbiotic Connection
This brings us to a fascinating point: the concept of “postbiotics.” Pasteurization kills the live microorganisms, but it leaves behind their byproducts—the short-chain fatty acids, organic acids, and other metabolites. These are known as postbiotics, and they can still confer health benefits to the host. The study results support this idea, suggesting that for blood pressure, at least, the postbiotic components of sauerkraut are doing the heavy lifting.
The researchers also found that the pasteurized sauerkraut seemed to have a more pronounced effect on some metabolic markers, like fructosamine (a measure of long-term blood sugar levels), though these changes were modest and largely confined to younger participants.
The Other Findings: Good News and Bad News
Aside from the blood pressure surprise, the study delivered a mixed bag of results for the “good news/bad news” column.
The Good News: The sauerkraut was well-tolerated. Most participants reported no negative side effects, and a significant number even reported some positive effects, like improved digestion or reduced feelings of hunger. This supports the idea that fermented foods are generally safe for consumption by the general public.
The Bad News (or the “It’s Complicated” News): The study did not find any significant improvements in the markers of inflammation or “leaky gut” that they were looking for. This means, in healthy individuals, eating sauerkraut for a month didn’t seem to reduce systemic inflammation (as measured by markers like IL-6 and hsCRP) or change the integrity of the gut barrier (as measured by markers like zonulin and lipopolysaccharide-binding protein).
The researchers did see some modest changes in inflammation markers (like hsCRP and TNFR-2) when they stratified by age or diet, but the overall effects were not robust enough to be considered a clear health benefit for everyone.
The Takeaway: Sauerkraut is Not a Magic Bullet, But It’s Not Nothing
A key takeaway from this study is the distinction between a food’s health-promoting properties and its role in disease treatment. The researchers concluded that “Four weeks of daily consumption was not linked to an appreciable systemic health benefit in healthy individuals”.
This is a crucial point. For someone who is already healthy, eating a daily portion of sauerkraut is likely not going to be a game-changer for their gut health or inflammation levels. The body, particularly the gut microbiome of a healthy person, appears to be quite resilient to short-term dietary changes.
However, this doesn’t mean the study is a wash. The small but real decrease in blood pressure is a positive finding, and one that is particularly interesting given the high sodium content. It suggests that even for healthy people, sauerkraut can contribute to a heart-healthy diet in a small but meaningful way.
Where sauerkraut may truly shine, however, is for individuals with chronic hypertension or those at elevated risk for cardiovascular disease. While the 1.5 to 2.5 mmHg reduction observed in healthy participants was modest, the effects could be more pronounced in populations with elevated baseline blood pressure—where there is more “room for improvement.”
For someone already managing high blood pressure, adding sauerkraut to their diet might complement existing lifestyle interventions like the DASH diet, regular exercise, and medication. Even small incremental reductions in blood pressure, when sustained over years, can meaningfully lower the risk of heart attacks, strokes, and other cardiovascular events.
The study’s findings are a promising signal that fermented foods like sauerkraut deserve a place at the table—not as a replacement for medical treatment, but as a supportive dietary ally for heart health.
