A Patient Case Study
A 61-year-old male patient came to my practice with a puzzling complaint. For months, he had been experiencing excessive abdominal gas, significant bloating, and discomfort that lasted throughout the entire day. Despite trying multiple interventions—eliminating FODMAPs, stopping coffee, avoiding resistant starch, and taking simethicone—the symptoms persisted.
What made his case particularly interesting was the connection to exercise. He reported that upper body movements, especially kettlebell swings and any activity involving his arms, would dramatically worsen his symptoms. The gas would become trapped, painful, and resistant to relief, and he would occasionally experience shortness of breath and a cold sensation in his biceps.
He had already seen his primary care physician about the pain and had undergone a thorough cardiac workup, which cleared him of any heart-related issues. However, despite being reassured that his heart was healthy, his digestive symptoms remained unresolved. Frustrated and seeking answers, he came to me for a fresh perspective.
His case was a diagnostic puzzle—until we examined his morning routine.
The Morning Ritual That Turned Against Him
Every morning, he would steep two teaspoons of dried hibiscus flowers in hot water and sip it throughout the morning on an empty stomach. He had read about hibiscus tea’s health benefits—antioxidants, blood pressure support, digestive aid. It seemed like the perfect way to start the day.
But something was wrong. Within hours of drinking the tea, he’d develop significant abdominal distention and gas that persisted until bedtime. The discomfort was constant, not just an occasional annoyance. The gas was trapped, painful, and resistant to relief.
What made it particularly strange was the exercise connection. He noticed that his morning workout—specifically, any movement involving his arms or upper body—would make the gas formation dramatically worse. Kettlebell swings, stationary running with arm movement, and even certain stretches seemed to amplify the problem. Standing and reaching overhead were particularly problematic.
At first, he suspected coffee. He stopped drinking coffee, but the gas persisted. He tried simethicone, which helped but took 20 minutes to work. He avoided FODMAPs. He eliminated resistant starch. Nothing fully resolved the issue until he finally asked: What if the hibiscus tea itself was the problem?
The Diagnosis: Hibiscus-Induced Gas and Bloating
After reviewing his dietary history, I identified the likely culprit: hibiscus tea. I recommended that he eliminate the tea entirely for one week to see if symptoms improved. The results were immediate and profound.
Within 24 hours of stopping the hibiscus tea, his gas and bloating began to subside. Within three days, all digestive symptoms had resolved completely. Perhaps most tellingly, he could suddenly tolerate coffee again—a beverage he had previously suspected but which was never the root cause.
The Science: Why Hibiscus Tea Causes Gas
So what’s happening inside the digestive tract? The answer lies in the unique biochemistry of Hibiscus sabdariffa.
1. Undigested Polyphenols Reach the Colon
Hibiscus is rich in polyphenols—plant compounds with powerful antioxidant properties. Research shows that a significant portion of these polyphenols resist digestion in the stomach and small intestine. Instead of being absorbed early in the digestive process, they travel intact to the large intestine.
Once in the colon, gut bacteria ferment these undigested compounds. Fermentation produces gas—hydrogen, methane, and carbon dioxide—as a natural byproduct. For people with sensitive digestive tracts or specific gut microbiome compositions, this fermentation can cause excessive gas production that persists for hours [1].
2. Enzyme Inhibition Slows Carbohydrate Digestion
Laboratory studies have shown that polyphenolic extracts from hibiscus can partially inhibit digestive enzymes like alpha-amylase and alpha-glucosidase—the enzymes responsible for breaking down complex carbohydrates into simple sugars [2].
When these enzymes are inhibited, carbohydrates reach the colon less digested than they should be. This provides additional fermentable fuel for gut bacteria, leading to even more gas production. It’s a double hit: the hibiscus compounds themselves are fermented, and they also make other foods more fermentable.
3. The Delayed Effect Explains the All-Day Discomfort
This mechanism explains why the gas effect lasted all day. The process—digestion resistance, colonic arrival, bacterial fermentation—takes time. Gas production doesn’t peak immediately after drinking the tea. It builds gradually throughout the day as the compounds slowly pass through the digestive system.
One study reviewing 26 clinical trials on hibiscus safety documented minor side effects, including abdominal pain and epigastric discomfort [3]. A systematic scoping review of safety data also identified adverse effects associated with hibiscus consumption, particularly with higher doses [4]. While these effects are considered minor and non-serious, they are real and can significantly impact quality of life.
4. The Dose Matters
My patient’s two-teaspoon morning dose was substantial. The research suggests that excessive intake—particularly strong tea consumed in large volumes—increases the risk of digestive side effects in sensitive individuals. Even a standard tea bag contains significantly less hibiscus than two teaspoons of loose flowers. He was essentially delivering a concentrated fermentation load to his colon every single day.
The Mystery of Upper Body Movement
The most puzzling aspect of this case was the connection between upper body movement and symptom severity. Why would swinging arms during kettlebell exercises or reaching overhead make gas formation worse?
The answer lies in the anatomy of the splenic flexure—a sharp bend in the colon located near the spleen, under the left ribcage. When gas builds up in the digestive tract, it can become trapped at this natural bend. The Cleveland Clinic describes this as splenic flexure syndrome, a condition where gas accumulating at this sharp curve causes bloating, fullness, and sharp upper abdominal pain [5, 6].
Upper body movement—especially reaching, stretching, or any motion that changes intra-abdominal pressure—can exacerbate this trapped gas. Think of what happens when you shake a sealed bottle of carbonated water: the gas already present becomes agitated and moves into new spaces, creating more pressure and discomfort.
Research on exercise-induced gas confirms that physical movement—particularly movements involving the torso and diaphragm—can mobilize gas that was previously trapped or settled [7]. Reaching overhead and swinging the arms engage core and shoulder muscles in ways that alter abdominal pressure dynamics, potentially shifting gas pockets and creating the sensation of worsened symptoms.
During exercise, the body also releases cortisol, which can lead to fluid retention and slowed digestion. When digestion slows, gas that should be moving through the colon gets trapped longer, worsening bloating and discomfort.
The diaphragm plays a role too. The phrenic nerve innervates the diaphragm, and any irritation or pressure affecting this area can influence both breathing and digestive function. When the diaphragm is compressed or agitated by gas or movement, it can create a feedback loop: more gas leads to more diaphragm irritation, which can lead to more symptoms.
The Takeaway Lessons
This case offers several important lessons about digestive health:
Lesson 1: “Healthy” is not universal. Hibiscus tea is a health-promoting beverage for many people. It has documented benefits for blood pressure, inflammation, and antioxidant status. But individual biochemistry varies tremendously. What nourishes one person’s gut may irritate another’s. This patient’s experience doesn’t make hibiscus tea “bad”—it makes it not right for him.
Lesson 2: Timing matters as much as content. Nutritionists often recommend drinking hibiscus tea after meals to leverage its mild laxative and digestive effects. The fruit acids in hibiscus can help combat heartburn, stomach cramps, and gas when consumed with food. But on an empty stomach—especially in large quantities—those same compounds can trigger distress. Empty stomach + strong tea = recipe for trouble.
Lesson 3: Dose is critical. Two teaspoons of hibiscus flowers is a substantial dose. A standard tea bag contains a fraction of that amount. The research on hibiscus safety highlights that adverse effects increase with excessive consumption. Starting with smaller amounts and observing your body’s response is always wise.
Lesson 4: Look for the “amplifier effect.” Hibiscus compounds can slow digestive enzyme activity, meaning they can magnify the effects of other foods. This explains why my patient could suddenly tolerate coffee after stopping hibiscus. The tea wasn’t creating gas from nothing—it was amplifying his existing sensitivity to other foods and beverages.
Lesson 5: Exercise can be a diagnostic tool. The connection between upper body movement and gas symptoms wasn’t random. It pointed to splenic flexure involvement, where gas trapped at a natural colon bend became agitated by movement. Physical activity can help relieve trapped gas in some cases, but for this patient, it was also revealing the location and nature of the problem.
Lesson 6: Elimination is the ultimate test. Simethicone, dietary changes, and other interventions all helped but never fully resolved the issue. Only eliminating hibiscus tea provided complete relief. If you’re struggling with unexplained digestive symptoms, consider a systematic elimination approach rather than treating symptoms individually.
Lesson 7: Listen to your body’s patterns. The fact that symptoms started after the tea, lasted all day, and worsened with specific movements were all data points. My patient initially dismissed the tea as the culprit because it’s considered “healthy.” But paying attention to timing, severity, and triggers eventually revealed the truth.
Treatment and Resolution
After eliminating hibiscus tea, my patient’s digestion returned to normal. The all-day gas resolved. He could exercise without discomfort. And he could once again enjoy coffee without consequences.
For those who want to reintroduce hibiscus tea, I recommend starting with small doses (1 teaspoon or less) and consuming it after meals rather than on an empty stomach. Monitoring symptoms and being willing to eliminate it again if problems return is the sensible approach.
Hibiscus tea remains a valuable beverage with legitimate health benefits. It just isn’t for everyone—and that’s okay. The most important health tool we have is attention to our own body’s responses.
Final Thoughts
This patient’s months-long struggle with all-day gas had a simple solution: stop drinking hibiscus tea. But discovering the solution required careful observation, understanding the science of digestion, and being willing to question an assumed “healthy” habit.
If you’re experiencing unexplained digestive symptoms, consider what you’re consuming—even if it seems harmless. Look for patterns in timing and triggers. Remember that what works for everyone else may not work for you. And trust what your body is telling you, even when it contradicts the conventional wisdom.
The tea stopped. The gas stopped. And the lesson became clear: health isn’t about following a checklist of supposedly healthy foods. It’s about finding what genuinely nourishes your body.
